Use of Cookies
The iMD Web site use "cookies" to help you personalize your online experience.
A cookie is a text file that is placed on your hard disk by a Web page server. Cookies
cannot be used to run programs or deliver viruses to your computer. Cookies are
uniquely assigned to you, and can only be read by a web server in the domain that
issued the cookie to you.
One of the primary purposes of cookies is to provide a convenience feature to
save you time. The purpose of a cookie is to tell the Web server that you have returned
to a specific page. For example, if you personalize iMD's pages, or register with
iMD site or services, a cookie helps iMD to recall your specific information
on subsequent visits. This simplifies the process of recording your personal information,
such as billing addresses, shipping addresses, and so on. When you return to the
same iMD Web site, the information you previously provided can be retrieved, so
you can easily use the iMD features that you customized.
You have the ability to accept or decline cookies. Most Web browsers automatically
accept cookies, but you can usually modify your browser setting to decline cookies
if you prefer. If you choose to decline cookies, you may not be able to fully experience
the interactive features of the iMD services or Web sites you visit.
Security of your Personal Information
iMD secures the information you provide on computer servers in a controlled,
secure environment that is protected from unauthorized access, use or disclosure.
When personal information (such as a credit card number) is transmitted to other
Web sites, it is protected through the use of encryption, such as the Secure Socket
Layer (SSL) protocol.
iMD has implemented measures to comply with the Safe Harbor
principles defined by the United States Department of Commerce. These principles
apply to the transfer of personal information from the European Union to the United
States.
Notice
Where iMD collects personal information directly from
individuals in the EU, iMD will inform individuals about the purposes for which
it collects and uses information about them, the types of third parties to which
it discloses the information and the choices and means the organization offers individuals

This notice will be provided in clear language in a conspicuous
manner at the same time individuals are first asked to provide personal information
to iMD, or as soon as possible thereafter, and in any event before iMD uses
or discloses information for a purpose other than that for which it was originally
collected.
Choice
iMD will offer individuals the opportunity to choose (opt-out)
whether their personal information is (a) to be disclosed to a non-agent third party,

For sensitive personal information, iMD will give individuals
affirmative and explicit choice (opt-in) to allow their information to be disclosed
to a non-agent third party or to be used for a purpose other than the purpose for
which it was originally collected or subsequently authorized by the individual.
iMD will provide individuals with reasonable mechanisms
to exercise their choices.
Onward Transfer
iMD will disclose personal information to third parties
pursuant to the Notice and Choice principles set forth above.
In the event that iMD wants to transfer Personal Data
to a third party that is acting as its agent, iMD will obtain assurances from
the third party that it:
- subscribes to the Safe Harbor Principles
- is subject to the EU Directive 95/46/EC
- is subject to another adequacy finding, or
- enters into a written agreement with iMD requiring that the third party
provide at least the same level of privacy protection as is required by the
relevant Safe Harbor Principles
Security
iMD will take reasonable precautions to protect personal
information collected from loss, misuse and unauthorized access, disclosure, alteration
and destruction.
Data integrity
iMD will seek to ensure that personal information collected
is processed in a way which is compatible with the purposes for which it was originally
collected or subsequently authorized by the individual. iMD will take reasonable
steps to ensure that the data is relevant to its intended use, accurate, complete
and current.
Access
Upon request, iMD will grant individuals reasonable access
to personal information possessed by iMD. In addition, upon request, iMD will
take reasonable steps to provide individuals with a means to correct, amend, or
delete information that is found to be inaccurate or incomplete.

iMD has put in place internal, self-assessment procedures
for periodic reviews of its relevant privacy practices to ensure compliance with
the principles set forth in this Policy.
Questions or concerns regarding the use or disclosure of personal information should be directed to Intelligent Medical Decisions, Inc. at imdprivacy@imedicaldecisions.com. iMD will investigate and attempt to resolve complaints and disputes regarding use and disclosure of personal information in accordance with the principles contained in this Policy.
In the event that a complaint cannot be resolved between
iMD and any complainant, iMD will cooperate and participate in the dispute resolution
procedures of the panel established by the EU data protection authorities to resolve
such disputes.
